Patient Navigator
Position Summary
The Patient Navigator serves as a compassionate guide for individuals diagnosed with cancer and their loved ones. This role provides personalized support throughout the cancer journey by helping clients navigate the healthcare system, identify barriers related to social determinants of health, and connect to appropriate resources and services. The Patient Navigator ensures clients feel supported, informed, and empowered every step of the way.
Key Responsibilities
Client Intake & Navigation
- Conduct comprehensive intakes with new clients to assess needs and determine appropriate support services
- Provide ongoing patient navigation throughout the cancer journey, helping clients understand treatment processes and available resources
- Maintain accurate and timely client records and documentation
Care Coordination
- Coordinate services between clients, healthcare providers, and community resources
- Assist clients in accessing support services such as transportation, food assistance, housing resources, and other needs related to social determinants of health
Resources & Financial Assistance Support
- Connect clients with financial assistance programs, grants, and community-based resources
- Provide information and referrals to local, state, and national cancer support organizations
Patient Education
- Provide educational materials and guidance to help clients better understand their diagnosis, treatment options, and available support services
- Help clients navigate the healthcare system, including appointments, referrals, and treatment processes
Emotional Support & Advocacy
- Offer compassionate emotional support to clients and their families
- Advocate on behalf of clients to help ensure they receive appropriate care and access to services
Program Support
- Assist with seasonal and special programs
- Help facilitate the Spanish Support Group and provide culturally responsive support to Spanish-speaking clients
- Participate in community outreach and educational events as needed
Qualifications
Required
- Bilingual in Spanish and English (verbal and written)
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Strong communication, organizational, and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information with compassion and professionalism
Preferred
- Certification as a Community Health Worker (CHW)
- Experience working with cancer patients, survivors, or within oncology support programs
- Knowledge of community resources and cancer support services
Skills & Competencies
- Compassionate and empathetic approach when working with clients
- Strong problem-solving and resource coordination skills
- Ability to work collaboratively with staff, volunteers, and community partners
- Cultural sensitivity and ability to work with diverse populations
